ClearCardio™ LLC is required by law to maintain the privacy of your protected health information (PHI), provide you this notice of our legal duties and privacy practices, and abide by its terms. We use HIPAA-compliant, BAA-supported systems for scheduling, communication and storage of health information.
We may use and disclose PHI for treatment (coordinating your care with our medical team), payment (billing and processing), and health care operations (quality and program improvement). We do not sell your PHI. We do not use PHI for third-party marketing.
You have the right to: inspect and obtain a copy of your PHI; request corrections; request restrictions on certain uses; request confidential communications; receive an accounting of disclosures; and receive a copy of this notice. To exercise any right, contact us at care@clearcardio.com.
Forms on this website collect contact information to schedule consultations. They are not intended for transmitting detailed medical information. Scheduling and any exchange of PHI happen exclusively through BAA-supported secure systems.
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
